Terms
Membership is an annual commitment and will be billed on the first of each calendar month. Early cancellation penalties apply. Memberships will auto renew unless canceled. 30-day cancellation notice required. Memberships cannot be transferred. No refunds will be offered for unused benefits. No substitutions unless arranged with written consent. Monthly and quarterly treatments cannot be banked or transferred and must be utilized within the calendar month or quarter, unless previously arranged and authorized. Cannot combine with special promotional offers other than Allé or Aspire Rewards. If a promotional offer is greater than member reward, member will be automatically granted promotional offer. To qualify for referral rewards, referral must be new patient to practice. Referral rewards are awarded upon completion of first service.
